At Concerned Home Care, Inc., our physical therapists start with an evaluation of the patient’s illness or injury. The evaluation is necessary to identify the patient’s rehabilitation potentials and needs in order to develop, implement, and manage a care plan based on the physician’s order, applying up-to-date home exercise techniques and procedures.
Physical Therapists will provide therapeutic procedures to either restore and/or improve range of motion, strength, balance, coordination, endurance, or functional ability. They conduct activities such as:
- Home Exercise Program
- Teaching self-care and maintenance programs to support long-term success following a surgical procedure, traumatic injury or for general health maintenance
- Using high-tech equipments such as Anodyne machine, electrotherapy units (Ultrasound, TENS, ES) to treat musculoskeletal and soft tissues injury, back and joint conditions, limited range of motion, and pain.
- Using assistive devices for gait training and safe transfers
- Training and treatments to promote the patient’s ability to become independent in their care
