A Message from Our CEO
Concerned Home Care, Inc. was launched in 1994 because of a need in the community. The continual growth and expansion of CHC has confirmed my initial assessment for the community needs by providing home setting teaching and health care.
As a clinic nurse, I recognized the health care needs of the elderly patients. At a busy physician’s clinic setting, there has never seemed to be enough time to teach them all they needed to know about medications, disease conditions, and other health education.
The agency started with four staff members. I did a lot of home care visits myself. I know the needs, the challenges, and the joys of providing good nursing care and other services like therapy and social work for our patients.
CHC now includes office staff, nurses, therapists, nurse aides, dietitian, a social worker, and a Medical Director. The office has been moved to different sites twice to provide extra space for home care service operations. In 2005, we opened a branch office in Fort Gratiot, Michigan.
Our agency is much more than an office. Like our motto, “Caring Hearts, Helping Hands,” our agency is about people. Our motto exemplifies the dedication of our employees who go far beyond what is being asked for.
At present, our patients require more than skilled nursing care. The coordination of services with the community is essential for a patient’s recovery. The home health care team at Concerned Home Care, Inc. is more than willing to be of service whether the patient needs aftercare following surgery, intravenous infusions, wound care, or extensive teaching regarding changes in medications or health care problems.
The demand in the community for provision of health care and teaching in the home setting is as great as, or greater today than it was a decade ago. As the life expectancy of the elderly increases, care for chronic illness is higher. It is with that challenge in mind that we at Concerned Home Care, Inc. strive and will continue its mission to deliver the best in-home health care services.
