What are the requirements for a patient to be eligible for home health services?
To qualify for home health services, one must be one or more of the following:
  • Homebound due to illness or disability
  • Under the care of a physician who will authorize home healthcare
  • In need of intermittent skilled services that can be provided at home
How can I choose Concerned Home Care?
Concerned Home Care is one of many State-licensed and Medicare-certified agencies to choose from in Michigan. When a patient is eligible and interested to receive home healthcare services, one must choose an agency that would provide services. Assistance in choosing an agency may be obtained from doctors, case managers, social workers, or even family and friends. Ultimately, the choice is that of the patient.

We service the counties of Huron, Sanilac, Tuscola, Bay, Saginaw, Genesee. Lapeer, St. Clair, and Macomb

What type of insurances and payment do you accept?
We accept a wide range of coverage options, including:
  • Medicare
  • Medicaid
  • Blue Cross Blue Shield
  • Private insurance
  • Private-pay
  • Other qualifying arrangements
What is covered by Medicare?
When a client qualifies for home care services, Medicare will cover 100% of home care services (i.e. Nursing, Physical Therapy, Home Health Aide, all other therapies, and Medical Social Services) and 80% of the approved amount for Durable Medical Equipment (i.e. wheelchair, hospital bed, walker, Glucometer). Medicaid (Public Aid) covers qualified clients for services as well. Most private insurances also cover qualified home care services.

How quickly can services be provided?
Generally services can be provided within a 24 to 48 hour period.
