	PT, PTA, OT, COTA, RN, and Registered Dietician We are currently seeking dynamic, experienced PT, PTA, OT, COTA, RN, and Registered Dietician  for our Home Care Agency.  Physical Therapist.  Evaluates patients, and plans and implements an effective physical therapy program based upon physical assessment, sound judgment and psychological sensitivity.  Must hold a current Michigan PT License. Occupational Therapist.  Will provide occupational therapy services to home care patients through comprehensive assessment, develop and implement plan of care consistent with Agency's mission.  Must hold a current Michigan OT license.  Physical Therapy/Occupational Therapy Assistant.   As part of an interdisciplinary team and under the supervision of an experienced PT/OT, perform restoration treatment plans for our clients in the comfort of their own homes.  Physical Therapy Assistant and Occupational Therapy Assistant will function and collaborate with other members of the health care team to provide optimum patient care.  Must be a Certified Occupational Therapy Assistant and Physical Therapy Assistant must hold an associate degree from an APTA accredited school. Registered Nurse.  Home care nurse provides nursing and support services to the elderly, chronically ill, and disabled clients to enable them to stay in their homes and to promote participation in activities of daily living.  Home care nurse ensures the delivery of quality nursing and support services to clients in their home settings, assisting them to function at their optimum level and remain independent of institutional based care services.  Must hold a current Michigan R.N. License.  Prior Home Care experience and knowledge of Medicare Certified Homecare regulations and paperwork, including OASIS, preferred.
