| Skilled Nursing |
CHC's licensed Skilled Nurses provide professional health
care and monitoring services in accordance with the plan of
care, under the direct supervision of the patient's physician.
Services include vital signs monitoring, wound care, blood draws,
medications, injections, IV therapy and rehabilitative nursing.
Nurses conduct teachings and training on various health care
procedures, such as but not exclusive to medication use and
interactions, wound care and diabetic education to the patient
and/or family members to promote self-care after nursing visit
are no longer required.

| Physical Therapy |
CHC's licensed Physical Therapist evaluate
a patient's rehabilitation potential and needs, applying up-to-date
tests and procedures. They will provide therapeutic procedures
to either restore and/or improve range of motion, strength,
balance, coordination, endurance, or functional ability. They
may conduct exercises, training and treatments to promote the
patient's ability to meet their maximum potential. Therapists
teach self-care and maintenance programs to support long-term
success following a surgical procedure, traumatic injury or
for general health maintenance.

| Occupational Therapy |
CHC's Licensed Occupational Therapists
evaluate the patient's level of function in their ability to
perform ADL'S. They help individuals with physical, cognitive
and visual deficits and assist them in returning to their optimal
level of function. The therapist educates the patient and /or
family on using compensatory strategies specialized rehabilitation
technique and adaptive equipment to increase their function
in tasks such as eating, bathing dressing and basic household
routines.

| Medical Social Work |
CHC's Medical Social Workers provide
assistance to the patient and family with social, emotional
and financial issues that are hindering the patient progress
towards recovery. They coordinate with appropriate medical and
community resources in an effort to supply the patient with
the necessary tools to meet their maximum medical and emotional
potential. Counseling services are also provided when the social
worker establishes the need for them. The social worker assist
patient's in enrolling in cost saving discount programs offered
by some, but not all, pharmaceutical companies. With senior's
cost of medication sometimes exceeding their monthly income, this
service becomes vitally important not only in providing the
patient's needs but also tending to their overall well being.

| Home Health Aide Services |
CHC's Home Health Aides provide help
with personal care, such as bathing, hygiene, dressing, grooming
and skin care. They also assist with meal preparations, medication
administration, feedings, ambulation, transfers and/or exercises.
These services are under the direct supervision of a nurse or
therapist as applicable.

| Speech Language Pathology |
CHC's Speech Language Pathologist is
dedicated to improving quality of life through the evaluation,
treatment and management of disorders of speech, voice, language
and swallowing.

| Nutritional Services |
CHC's Registered Dietitian provides counseling
services to individuals who need professional dietary assessment
and guidance to properly address a dietary requirement in order
to manage an illness or disability.

| Private Pay Services |
There are times when insurance is not
available to cover the expenses of personal care, meal preparations,
assisting with ambulation, exercises and so on. It is at these
times that our qualified professional home health aides can
provide these services at an affordable price.
