Concerned Home Care, Inc. was
launched in 1994 because of a need in the community. Our continual
growth and expansion of CHC has confirmed my initial assessment for
that community need for providing home setting teaching and health
care.
As a nurse working in my husband's office, I was close to so many
senior citizens and never seemed to be enough time to teach them all
they needed to know about medications, wound care, and other health
education.
We were a staff of four back then and I did a lot of home care visits
myself. I know the needs, the challenges and the joys of providing
good nursing care and other services like therapy, and social work
for our patients.
CHC now includes office staff, nurses, therapists, nurse aides, dietician
and a social worker. Dr. Jaime Tan serves as medical director. The
Office has been moved to different sites twice to provide extra space
for home care service operations; and in 2005, we opened a second
office in Fort Gratiot, Michigan. But, our agency is much more than
an office.
Like our motto, "Caring Hearts, Helping Hands," our agency
is about people. Our motto exemplifies the dedication of our employees,
who go far beyond what's asked for; they sacrifice for their patients.
Our patients served today require more skilled nursing care and coordination
of services in the community. Whether the patient requires after care
following surgery, intravenous infusions, wound care or extensive
teaching related to changes in medications or health care status,
staff at Concerned Home Care, Inc. are anxious to be of service.
That need in the community for provision of health care and teaching
in the home setting is as great or greater today than it was more
than a decade ago. It is with that challenge in mind that we at Concerned
Home Care, Inc. will strive to continue to deliver the best in home
health care services.
