Concerned Home Care Inc. is a home health care agency that offers services to patients in the state of Michigan living in Huron, Sanilac, Tuscola, Bay, Saginaw, Genesee, Lapeer, St. Clair and Macomb counties. Concerned Home Care services include skilled nursing, physical therapy, occupational therapy, medical social work, diabetic care and home health aide. We provide all of our services in the privacy and comfort of your own home or alternate residence. These services are provided to promote the clients ability to remain in their own home during times of medical crisis. Many insurance companies including Medicare, Medicaid, Blue Cross, and other insurances will pay for these services if appropriate criteria are met.

Concerned Home Care is dedicated to improving the quality of life for its patients and their families. It is our belief that cost effective, outcome-based care can be delivered successfully without compromising patient respect and self-dignity. We aspire daily to fulfill this belief.
